Qualifying Patient Extension Instructions

Extension Instructions

  1. If you want to continue with the MCPP program, submit an extension prior to the card expiring. Refer to the Qualifying Patient Renewal instructions for the renewal process. An email will be sent to you when it is time to extend your card. Sign into the system using your username and password.
  2. Select the application in the Tracking Inbox with the Pending Extension status.

Completing the Extension

  1. If you are changing your name, address, or driver’s license/state ID information, submit a change of information by going to the “Change of Information” tab.
  2. If you have only had your MCPP card for 1 year, select the timeframe for 1-year or 2-years term card.
    You will also have to answer the caregiver question. If you currently have a caregiver(s) and select Yes to the caregiver question, all current caregivers will be extended. If you would like to extend one caregiver’s card and cancel another caregiver’s card, select Yes. If you would like to cancel the future card for your current caregiver(s) or you are not adding a caregiver during the extension process, select No.
  3. You need to answer the reduced fee questions during the extension. In the Fees section, if you answer Yes to either question, you will be required to upload the files.
  4. Once your information is complete, select Save. If you are canceling a caregiver or adding caregivers during the extension process, DO NOT SELECT Continue yet. Go to the MCPP Patient Caregiver tab.

    If you are extending your caregivers or do not have any caregivers, select Save and then Continue to progress to the payment section.

Cancelling or Adding a Caregiver in the Renewal Application

  1. Go to the MCPP Patient Caregiver tab.
  2. If you would like to cancel the future card of your current caregiver, select the caregiver from the tracking inbox.
  3. Once you open the caregiver application, select Cancel Caregiver at the bottom of the page. A warning message will pop up. Click OK to cancel the caregiver.


    Once this is selected, the system will take you back to the caregiver list. If you select the canceled caregiver, you will see a message that the future card is canceled.

    If you would like the current caregiver card canceled while you are in Pending Extension status, send an email request to DPH.MedicalCannabis@illinois.gov. Include your name, date of birth, QP number, caregiver name, caregiver date of birth, and caregiver number in the cancel request.
  4. If you would like to add a caregiver while in Pending Extension status, select + New on the MCPP Patient Caregiver tab and complete the fields marked with an asterisk and upload the required documents. If you answer Yes you want to waive the caregiver photo, click on the Photo Affidavit link to download the form, complete the form with the caregiver information, and upload the form into the application. If you answer No, upload a photo of the caregiver. (Must be in JPEG format).

    If your registration is Pending Extension status, your added caregiver will become effective with your extension card. The fee is $25 a year.
    Important Notice: If you add a caregiver(s) during your Extension Application, you will only be charged a $25 fee for the current card. A separate application fee will be charged for the Extension Application and include any caregiver (s) added for the future card.
    In the Caregiver Proof of Identity section, click on the dropdown box and select the Identification Type (state driver’s license, state ID card, passport, or other). You will be required to upload a document for proof of identification based on your selection.
    1. If you select state driver’s license for the Identification Type, you will be required to select the state of issuance from the dropdown list. If you select IL, you will be required to enter the driver’s license number and answer the proof of residency question: Does the address on your driver’s license/state ID match the address listed on the application? If you answer No to this question, you will be required to upload a document for proof of residency. If you select other state driver’s license, you will be required to upload a document for proof of residency.
    2. If you select state driver’s license for the Identification Type, you will be required to select the state of issuance. If you select IL, you will be required to enter the driver’s license number and answer the proof of residency question: Does the address on your driver’s license/state ID match the address listed on the application? If you answer No to this question, you will be required to upload a document for proof of residency. If you select other state driver’s license, you will be required to upload a document for proof of residency.
    3. If you select passport for the Identification Type, you will not be required to answer the proof of residency question: Does the address on your driver’s license/state ID match the address listed on the application? However, you will be required to upload a document for proof of residency.
    4. If you select other for the Identification Type, you will not be required to answer the proof of residency question: Does the address on your driver’s license/state ID match the address listed on the application?” However, you will be required to upload a document for proof of residency. This option is only available to individuals without any of the above-mentioned proof of identification documents.
      Note: Approval of alternative forms of identification is up to the Illinois Department of Public Health’s discretion.


      Once you have completed the caregiver application, select Save. If you selected to add the caregiver to your current card, you are taken to the payment tab to pay the caregiver fee for the current card. After the caregiver fee is paid for the current card, the system will take you to the Patient Application page. If you need to add or cancel any additional caregivers, go back to the MCPP Patient Caregiver tab.
      If your caregiver applications are complete, select Save and click on Continue at the bottom of the MCPP Patient Registration page to progress to the Awaiting Extension Payment status.

Extension Payment

  1. To make a payment, select and click + New.

  2. Select Click Here to Pay to finalize the application. After the payment is submitted, you will receive an email notification.
