Qualifying Terminal Minor Patient Application Instructions

Initial Patient Registration

  1. Minor caregivers can register to the Illinois Cannabis Tracking System (ICTS) at https://etk.icts.illinois.gov/etk-icts-prod/login.request.do and select MCPP Patient Registration.
  2. To register, the minor caregiver must complete all the information and click Register.
  3. Once the minor caregiver successfully completes the registration process, an email notification will be sent with a temporary password.
  4. The minor caregiver will be asked to change the password with the first log-in.
  5. Change the temporary password and click on Save.

Starting MCPP Minor Terminal Patient Application

  1. New Applicants: Select + New MCPP Patient Registration to begin a MCPP application. If you are not sure which program to apply for, talk to your health care provider before starting the application.
  2. Select I am applying on behalf of my minor child in the Application Type.
  3. Answer Yes to the terminal illness question.
    Note: If your child has not been diagnosed with a terminal illness or disease by a health care provider, refer to the Qualifying Minor Patient Application instructions.
  4. In the Patient Information section, complete the required fields marked with an asterisk and upload the required documents. Verify the patient information is correct and the required documents are uploaded before you click Save.
  5. After clicking Save, proceed to the MCPP Patient Caregiver tab to add a caregiver.
    Note: By law, Qualifying Minor Patient Applications must have at least one caregiver who is a parent or legal guardian.

Starting MCPP Terminal Minor Caregiver Application

  1. To add a caregiver, go to the MCPP Patient Caregiver tab and select + New to begin the caregiver application. Before adding a caregiver, make sure you answered Yes to the caregiver question in the Patient Application section.
    Note: A patient can have a maximum of three caregivers.
  2. In the Caregiver Information section, complete the required fields marked with an asterisk. Upload the required documents based on your responses. If you answer Yes to the photo affidavit, click on the Photo Affidavit link to download the form, complete the form, and upload the form. If you answer No,’” upload a photo of the caregiver (must be in JPEG format). These steps will have to be completed for each caregiver added.
    Note: The photo must not be a picture of the caregiver(s) driver’s license, state ID card, or government issued passport.

  3. For the caregiver proof of identity, click on the dropdown box and select the Identification Type (state driver’s license, state ID card, passport, or other). You will be required to upload a document for proof of identification. Based on your selection, you will be required to upload a document for proof of residency or/and answer the proof of residency question: “Does the address on your driver’s license/state ID match the address listed on the application?”
    1. If you select state driver’s license for the Identification Type, you will be required to select the state of issuance from the dropdown list. If you select IL, you will be required to enter the driver’s license number and answer the proof of residency question: “Does the address on your driver’s license/state ID match the address listed on the application?” If you answer No to this question, you will be required to upload a document for proof of residency. If you select other state driver’s license, you will be required to upload a document for proof of residency.
    2. If you select state ID card for the Identification Type you will be required to answer the proof of residency question: “Does the address on your driver’s license/state ID match the address listed on the application?” If you answer No to this question, you will be required to upload a document for proof of residency.
    3. If you select passport for the Identification Type, you will not be required to answer the proof of residency question: “Does the address on your driver’s license/state ID match the address listed on the application?” However, you will be required to upload a document for proof of residency.
    4. If you select other for the Identification Type, you will not be required to answer the proof of residency question: “Does the address on your driver’s license/state ID match the address listed on the application?” However, you will be required to upload a document for proof of residency. This option is only available to individuals without any of the above-mentioned proof of identification documents.
      Note: Approval of alternative forms of identification is up to the Illinois Department of Public Health’s discretion.


  4. After completing the Caregiver Identification section, answer the question “Is this caregiver the parent of the minor qualifying patient?” If you answer Yes, no documents will be required to be uploaded in this section. If your name is not listed on the birth certificate, you need to upload the appropriate documentation by clicking Browse.
  5. If you answer No to the question “Is this caregiver, the parent of the minor qualifying patient?”, you will need to answer the question “Is this caregiver the legal guardian of the minor qualifying patient?” If you answer Yes, you will need to upload legal guardianship documentation.
  6. If you answer No to the question “Is this caregiver, the legal guardian of the minor qualifying patient?”, you must state a reason why the individual shall be a caregiver for this minor.
  7. In the Caregiver Consent Upload section click on Click Here to download the Caregiver Consent Form. Once the form has been completed, proceed to upload the completed document and click Save.
    Note: Before you click Save, verify the information is correct and the required documents have been uploaded for the caregiver.
  8. After you have completed the required fields, upload the required documentation, and verify the information is correct, click Save. Once you have added all your caregivers, click on the MCPP Patient Registration tab and click the Continue button to continue the application process.

Health Care Provider Certification Upload

  1. To add a health care provider certification, click the Provider Certification tab and click on the + New button.
    NOTE: Health care provider’s certifications are only valid for 90 days from the date of in-person/telemedicine examination. Minor qualifying patients require only one health care provider certification (Recommending Certification).
  2. Once you have selected the health care provider certification, click Save.
    Note: The health care provider certification will only be available to upload for the patient once the health care provider has submitted the certification. If no certification is available to upload, contact your health care provider.
