Illinois is currently accepting applications for COVID-19 vaccine clinics outside of the City of Chicago to improve equity of vaccine distribution (the City of Chicago is directly running clinics within City limits). Use this link to submit an application to host a clinic in your community.
This initiative will help to ensure equity in vaccine distribution to underserved racial/ethnic minorities and other groups that have experienced challenges accessing vaccines. The purpose of these clinics is to collaborate with community partners to help overcome barriers to vaccination such as language, technology, disabilities, transportation, clinic location, and hours.
PLEASE NOTE: We cannot accept locations within the City of Chicago. This application is for Community Partners that want to host a clinic for underserved racial/ethnic minority and other groups that have experienced challenges accessing vaccine.
We will not accept applications from vendors. The Illinois Department of Public Health (IDPH) is currently accepting applications for COVID-19 vaccine clinics in the State of Illinois (excluding the City of Chicago) to improve equity of vaccine distribution.
Please note this is a limited resource so the IDPH Office of Minority Health and IDPH Vaccination Program will not be able to respond to all requests.
