What is the process if the Rural Health Center (RHC) lost its Mid-level provider?

The RHC shall submit a letter to the Illinois Department of Public Health with an effective date of the loss of your Mid-Level Practitioner. The Rural Health Clinic guidelines state that the agency must comply with the staffing requirements within 90 days from the date it informed the Illinois Department of Public Health that it no longer met the staffing requirements, or be terminated unless a waiver request is submitted by the facility and approved by the Centers for Medicare and Medicaid Services prior to the end of the 90-day period.  Please review the attached Rural Health Clinic’s One Year Waiver Addendum, which outlines information regarding the waiver process, and states various types of documentation that must be provided to the Department of Public Health in order to apply for this waiver.

What is required for a relocation of the health care provider?

The health care provider shall complete the IDPH “Facility Information Change Form” prior to the relocation and submit to the Department.  The Health Care Provider shall also complete Medicare Provider enrollment form A and submit to their Medicare fiscal intermediary.
